Traditionally, the gold standard for cancer diagnosis has been tissue biopsy, which involves taking a tissue sample from the affected area. However, tissue biopsies are invasive and can be associated with complications such as bleeding and infection. Moreover, cancer is a dynamic disease, and the genetic profile of a tumor can change over time, leading to treatment resistance and disease recurrence. Therefore, there is a need for non-invasive diagnostic approaches that can detect cancer early and monitor its progression and response to treatment.

Liquid biopsy is a non-invasive diagnostic approach that uses biofluids such as blood, urine, and cerebrospinal fluid to detect cancer. The technique involves the isolation and analysis of circulating tumor cells (CTCs), cell-free DNA (cfDNA), and other biomarkers present in the biofluids. The analysis of these biomarkers can provide information about the genetic profile of the tumor, its stage, and its response to treatment.

Liquid biopsy is a game-changer in cancer detection and monitoring for several reasons. Firstly, it is non-invasive and can be performed repeatedly, allowing for real-time monitoring of disease progression and treatment response. Secondly, it can detect cancer at an early stage, even before the tumor is visible on imaging scans. Thirdly, it can detect the genetic heterogeneity of the tumor, providing a more comprehensive understanding of the disease than a single tissue biopsy can provide. Finally, it can overcome the limitations of tissue biopsy, such as the risk of complications, and the difficulty of obtaining tissue samples from certain parts of the body.

One of the most promising applications of liquid biopsy is in the early detection of cancer. Cancer is most treatable when detected early, but currently available screening methods, such as mammography and colonoscopy, have limitations. Liquid biopsy has the potential to overcome these limitations and detect cancer early. Studies have shown that liquid biopsy can detect early-stage lung cancer with a sensitivity of over 80%, and early-stage ovarian cancer with a sensitivity of over 90%.

Liquid biopsy can also be used to monitor cancer progression and treatment response. Cancer is a dynamic disease, and its genetic profile can change over time. Tissue biopsies are invasive and can only provide information about the tumor at a specific point in time. In contrast, liquid biopsy can be performed repeatedly, allowing for real-time monitoring of the genetic changes in the tumor. Another potential application of liquid biopsy is in the identification of minimal residual disease (MRD). MRD refers to the small number of cancer cells that remain in the body after treatment and can lead to disease recurrence. Current methods for detecting MRD, such as imaging scans and tissue biopsies, have limitations. Liquid biopsy has the potential to overcome these limitations and detect MRD with a higher sensitivity and specificity.

Liquid biopsy is still an emerging technology, and there are several challenges that need to be overcome before it can be widely adopted in clinical practice. One of the main challenges is the low abundance of CTCs and cfDNA in biofluids, which can make their isolation and analysis challenging. Another challenge is the lack of standardization in the isolation and analysis of biomarkers. Standardization is essential to ensure the reproducibility and accuracy of liquid biopsy results.
